ABSTRACT

Introduction: Rubber band ligation is a minimally invasive outpatient hemorrhoid treatment with low cost, low complication rates, and rapid realization. It is performed with the aid of an anoscope and uses a rubber ring that surrounds the hemorrhoidal nipple, causing compression of the vascular structures of the tissue, leading to necrosis and remission of the hemorrhoid. No device for training this essential procedure for treating this pathology has been identified in the literature. Therefore, we aim to develop a low-cost simulator for training hemorrhoidal rubber ligation. Methods: The model was constructed using PVC pipe wrapped in neoprene fabric. Hemorrhoidal nipples and the pectineal line were also simulated using fabric and sewing threads. The procedure is performed with conventional anoscope and ligature forceps. Conclusion: The device in question is a low-cost simulation model designed to train the skills required to perform a rubber band ligation and review the basic anatomy of the anal canal during anoscopy. Given these qualities, the model can be used for academic training due to its low cost and simplicity of application. (AU)

ABSTRACT

Early diagnosis of autism spectrum disorder (ASD) is essential for improved outcomes. There is a paucity of data on the prevalence of ASD in low- and middle-income countries (LMIC), but early identifcation may be further delayed in those communities. In this paper, recent studies on strategies for the early detection of ASD, and the prevalence of ASD in LMIC are reviewed. The limitations that can arise in the early identifcation of ASD in LMIC communities are discussed, and screening tools and strategies that can be helpful are identifed. The goal is to recommend models that are culturally appropriate and scientifcally valid, easily integrated within community settings while strengthening community systems and reducing disparities in the early identifcation of ASD. Starting locally by simplifying and demystifying the ASD identifcation process and building community connections will inform global researchers and policymakers while making a diference in the lives of the children and families afected by ASD.

ABSTRACT

Abstract: Introduction: In the face of globalization, crisis scenarios and unpredictable situations, there is a need for the development of leaders capable of returning situations to their normality. In this scenario, authentic leadership presents an innovative proposal, based on the knowledge, skills and attitudes of the life of the leader. Part of this triad, the skills, are divided, theoretically, into hard skiils (technical skills) and soft skills (personal skills and relationship). In the context of nursing work, nurses occupy a prominent place, going through the dimensions of management and care, needing to improve these skills. Because it is authentic leadership, a process of intimate expression of who we are, so that such skills can be improved, it is necessary to develop self-awareness through processes of self-assessment and self-development. Objective: To develop and validate a self-assessment instrument of personal skills for authentic leadership in nursing. Method: Methodological study, which aims to construct and validate a self-assessment instrument of personal skills for authentic leadership in nursing, whose given name was "LEADERSKILLS". The content was validated using the Delphi Online technique. It was considered as Content Validity Index (CVI) >=0.85 in the first round and CVI >=0.80 in the second round, based on the number of experts and the characteristics of the construct. They were part of the panel of professional experts with at least five points of the criterion for the selection of experts of Joventino (2010). The first round of 13 nursing professionals from all regions of Brazil and the second, 5 professionals participated in the first round. The evaluations took place in the first round, via survey monkey® platform, and the second by the participants' response emails, totaling two rounds of consensus. Results: The first stage of construct elaboration allowed to generate an outline of the instrument that was submitted to pre-test to the participants of the research group. This pre-evaluation allowed us to observe the need to improve visual aspects, which underwent modifications with the graphic designer. The result of these changes generated the first version of the instrument that was forwarded for evaluation by the judges. Four categories of data were evaluated, such as: content, language, layout, motivation and culture. Within these items, 17 multiple choice questions and two dissertationquestions were elaborated. Of the total number of questions evaluated, only two (question 11 and question 17) did not reach consensus in the first round. The second round of consensus culminated in the validation of the content of the "LEADESKILLS". Conclusion: Thus, the research allowed to achieve the proposed objectives, corroborating the literature, which signals the scarcity and need to study the theme of soft skills. Thus, the construction of this instrument will be an adjunct to self-knowledge and self-development, since it will foster the debate of nursing professionals.

ABSTRACT

ABSTRACT Background: training in critical surgical situations is crucial for a safe outcome. The use of simulators is well established, although many are quite expensive, requiring the search for financially viable solutions for training centers. Methods: we built a low-cost simulator for intra-abdominal bleeding with inexpensive materials, such as a manikin chest, latex tubes, silicone rubber, and waterproof fabric, seeking to mimic the abdominal viscera and vessels and their anatomical correlations. An IV infusion set allowed simulated blood to flow under pressure, and the blood flowed freely during simulation. After obtaining a functional model, we selected general surgeons to validate the simulator and its use in teaching surgery. We used the content validity index (CVI), with a cutoff of 0.9. Results: the cost of building the prototype was US$71,00 in 2021, accounting for the purchase of the various necessary materials. Twelve raters participated in the validation tests. The results obtained from the feedback survey showed a good evaluation of all items, especially the recognition of the injured vessel, access to the vascular injury, hemostasis by manual compression, and hemostatic suturing. Conclusion: the proposed simulator obtained good results in scenarios of intra-abdominal bleeding from large vessels, as well as for hemostasis by manual compression and suturing. It proved to be a useful tool for training in critical intra- abdominal bleeding situations, while maintaining a low cost of building.

ABSTRACT

In the last 30 years soils decreased 73 % of their productive capacity in the world. One of the main reasons for this decrease has been the politics implemented, as well as the absence of opportunity for the small producer to implement improvement strategies that can increase their productivity in the field. Therefore, in this research a soil measurement system is proposed for small farmers, based on the design of two sensors of low cost: (i) Apparent electrical conductivity (ECa) sensor and (ii) moisture sensor. These measurement variables have the particularity that their characteristics can be associated with the physical properties of the soil for decision-making. The sensors developed in this work employ two techniques: Wenner's method for ECa and electrical impedance measurement to identify the dielectric permittivity associated with the water content of the soil. The system is controlled by the Raspberry Pi 3 board through developed software that allows correlating data with its geolocation. The device was tested in controlled laboratory measurements with calibrated instruments, obtaining a coefficient of . Finally, on-site tests were made to validate their use for small farmers.

ABSTRACT

Abstract The present study created a cheap (below US$ 100) shoulder arthroscopy training model, affordable for the practical education of medical students and residents. The model was created using a polyvinyl chloride (PVC) knee joint pipe (150 mm in diameter and 90 degrees in inclination) and a synthetic shoulder model. The parts were arranged to simulate a lateral recumbency with the upper limb in traction, which is the frequent positioning during arthroscopies. Colored dots on the glenoid and a partial rotator cuff model on the upper portion of the scapula were placed to assist training. This inexpensive, easy-to-make model for shoulder arthroscopy can aid surgical training.

ABSTRACT

Objective@#Neurosurgical boot camps allow trainees to hone practical skills in a risk-free environment, but the models and simulators used are relatively costly. In developing countries like the Philippines, low-cost alternatives have to be devised. The authors aimed to demonstrate the feasibility of using local, readily available, and inexpensive tropical fruits as surrogate models for basic neurosurgical skills training during a neurosurgical boot camp.@*Methods@#Locally available tropical fruits were used to teach basic neurosurgical skills to trainees. Coconut, pomelo, and watermelon were used as models for head clamp application, scalp and dural suturing, and ventriculostomy, respectively. Feedback was obtained from the participants after the boot camp.@*Results@#All eight residents thought that the boot camp was useful in learning new skills, and that the fruit models served their purpose. The trainees favored the fruit models that catered to the skill sets required according to level of training. The use of tropical fruits in the boot camp also provided an informal atmosphere that was conducive to learning. @*Conclusion@#The novel use of tropical fruits as surrogate models in basic neurosurgical skills training was a feasible and affordable alternative in resource-limited settings, although the activity was perceived to be more useful to junior than to senior residents. The informal atmosphere generated by the use of the fruits contributed to an improved learning experience for the trainee.

ABSTRACT

Background: Persons living with HIV (PLWH) are more likely to develop hypertension and cardiovascular disease than the HIV-negative population. The new hypertension guidelines by the American Heart Association (AHA) and the American College of Cardiology (ACC) lowered the definition of hypertension from systolic and diastolic blood pressure (BP) of ≥ 140/90mmHg to ≥ 130/80, respectively. This study was aimed at determining the prevalence and factors associated with hypertension in PLWH in Livingstone using the new hypertension diagnostic criteria. Methods: This was a cross-sectional study. We recruited 226 antiretroviral treated PLWH attending routine visits. Socio-demographic, health and clinical data including BP readings were collected. Interviewer-structured questionnaires adapted from the World Health Organization Stepwise approach to Surveillance ( WHO STEPs) and the international physical activity questionnaire (IPAQ) were used to collect data. Statistical evaluations were employed to elucidate relationships between hypertension and all response variables. Results: The prevalence of hypertension using the old and new guidelines was 16% and 42%, respectively. Factors significantly associated with increased and reduced odds of developing hypertension after adjustments in multivariate logistic regression were age, body mass index (BMI), employment status, fasting blood sugar (FBS) and table salt consumption, respectively (p<0.05 for all). Using the new AHA/ACC criteria for hypertension shifted the prevalence from 16% (old criteria) to 42%.Conclusion: The prevalence of hypertension in PLH in Livingstone was 42% and the major risk factors associated with hypertension in PLWH were increasing age, BMI and FBS. We recommend the inclusion of FBS in routine measurements in PLWH. The AHA/ ACC new guidelines should be reenforced in low-cost settings to increase the treatment of hypertension among PLWH.

ABSTRACT

Background: Respiratory distress syndrome causes significant morbidity and death especially among very low birth weight babies. Though the use of CPAP and surfactant have been shown to improve survival, these interventions were scarcely available in the past. This study aimed at comparing the clinical outcomes of preterm babies with RDS delivered at the Ife Hospital Unit of the Obafemi Awolowo University Teaching Hospitals Complex at two different periods. Objective: To compare the birth weight specific mortality rates and overall mortality rates of preterm babies with RDS between two periods in the neonatal ward of the Ife Hospital Unit of OAUTHC. Methods: A retrospective study comparing outcomes of 92 babies with RDS at GA 26 to 33+6 weeks between January 2015 and May 2016 and managed with intranasal oxygen alone to 104 babies of same gestational age characteristics between January 2019 and May 2020 who were managed withCPAP/surfactant. Results: The mean weight and gestational age of the babies respectively were 1.36 (±0.37) kg and 31.14 (±2.3) weeks in 2015/2016 and 1.35 (±0.322) kg and 30.95 (±2.24) weeks in 2019/2020. The overall case fatality rate and birth-weight specific mortality rates for ELBW, VLBW and LBW were 33.7%, 62.5%, 35.2% and 9.1% in 2015/2016 and 18.3%, 58.3%, 15.5% and 9.7% respectively in 2019/2020. Conclusion: While the use of CPAP and the administration of surfactant clearly show improved survival among very low birth weight babies who are at increased risk of death from RDS, this was not the case for extreme low birth weight babies.

ABSTRACT

ABSTRACT Red mud, a waste product generated during alumina extraction from bauxite, could be used as a low-cost adsorbent. Here, the effect of thermal treatment on the adsorption of Reactive Blue 19 (RB19) dye by red mud was compared with the adsorption capacity of untreated red mud. Thermal treatment of red mud at 500°C results in an increase in adsorption capacity from 357 mg g-1 (untreated red mud) to 416 mg g-1, under acidic conditions. Red mud samples thermally treated at 600°C and 800°C show a reduction in adsorption capacity, however, falling to 337 mg g-1, in acid medium. The change in the maximum adsorption capacity of red mud to RB19 following thermal treatment is associated with specific surface area. Red mud subjected to 500°C can be used for the treatment of water and wastewaters with a higher efficiency than untreated red mud, thus finding possible application in the textile industry.

ABSTRACT

After a visit to a hydrotherapy laboratory, limited resources for use in lower limbs were noted. For this reason, the goal is to design a water bike from low-cost material. The process took place in three phases: (1) project carried out, (2) prototype construction and (3) bicycle construction. The confection was divided into six parts; the materials used were polyvinyl chloride (PVC) tubes for the construction of sewer system. In addition to PVC materials, other products were used: two plastic pedals; a gel saddle, a seat post, two cranksets, a pipe clamp, a central shaft, two industrial bearings, three 40-millimeter aluminum cylinders and four suction cups. The final product was measured 69 centimeters (cm) in width, 59.05 cm in length; as the seat can be adjusted, the height varies. The prototype ranges from 73 to 102 cm in height and supports up to 85 kilograms (kg). It is concluded that although there is a limitation as to the design of the final product due to the limited formats of the PVC connections and as to the adaptations that must be made so that the final product can support a considerable weight, it is feasible to make a low-cost water exercise bike using PVC as the main material that can be used in the pool. It is noteworthy, however, that this product will still be tested to ascertain functionality as for equipment for clinical practice. (AU)

(AU)

ABSTRACT

Abstract Introduction: Heart exploration is an essential clinical competence that requires continuous training and exposure. Low availability and accessibility to patients with heart disease constitutes a barrier to acquiring this competence. Inadequate cardiac auscultation skills in medical students, residents, and graduate physicians have been documented. Objective: To develop and validate a low-cost, high-fidelity simulator for heart exploration. Methods: A low-cost, high-fidelity heart examination simulator capable of reproducing normal cardiac sounds was designed and developed. Subsequently, the simulator was validated by a group of experts who gave their opinion according to a Likert scale. Results: Ninety-four percent agreed that the simulator motivates the learning of heart exploration, and 92 % considered it to be a realistic model; 91 % considered that the simulator is an attractive tool to reinforce learning and 98 % recommended its further use. Conclusions: The use of the simulator facilitates the acquisition of skills and stimulates learning in the student, which can be attributed to repeated practice, longer exposure time and cognitive interaction.

ABSTRACT

ABSTRACT: A synthetic medium containing glucose, glycerol, yeast extract (YE), and ammonium sulfate (AS) was compared to several low-cost media in their ability to produce high emulsification index (EI). The goal was to reduce the production costs of an emulsifier with application in food oil-in-water emulsions. To this end, agro-industrial by-products were screened for bioemulsifier production from Yarrowia lipolytica. The statistical analysis showed that the EIs of media containing residual frying oil from palm oil (RFO_palm) or soybean oil (RFO_soy), residual liquid from butter production (butter whey, BWhey) or cheese production (cheese whey, CWhey), supplemented with YE and AS were similar to the EI of the synthetic medium. The replacement of YE by corn steep liquor (CSL) also resulted in similar EI, except for RFO_soy. BWhey was tested with CSL without AS and similar EI (66.8%) was detected in comparison to that of the same medium with AS (66.3%). The cell-free broth obtained after Y. lipolytica growth in BWhey+CSL was successfully used to obtain vegetable oil-in-water emulsions indicating its potential application in food products.

ABSTRACT

Abstract Current trends in science education recommend the complementary use of virtual and hands-on methods of teaching. In neurobiology, for instance, there is a plethora of virtual laboratories and simulators that can be readily combined with traditional physical labs. Unfortunately, physical laboratories are almost unaffordable for many institutions due to the high cost of equipment. In this paper, we present a simple and low-cost in vivo method for demonstrating some of the basic biophysical properties of neural action potentials. The method involves the following steps: a) dissection of the ventral nerve cord of earthworm; b) electrical stimulation; c) amplification and visualization of the medial and lateral giant fibers' action potentials; and d) recording. The system showed stability, expected amplification, high signal-to-noise ratio, and an estimated total cost of US$ 5.662. We provide guidelines for assembling the system and discus its utility as a teaching alternative for low budget institutions.

ABSTRACT

Introduction: In hemiparesis the inadequate weight-bearing in the lower limbs generate balance deficits, and balance planks are essential for evaluation. Objective: To verify the clinical applicability of a low-cost board and the Berg Balance Scale (BBS) in the assessment of balance and weight distribution in the lower limbs of hemiparetic patients. Methods: A pilot, cross-sectional, quantitative, technological innovation study carried out an evaluation of 21 individuals affected by stroke with a low-cost plank. Results: Ischemic stroke and right hemiparesis were predominant. In the evaluation with the plank, participants with right hemiparesis unloaded more weight in the hindfoot region, with both feet together and hip width, while those with left hemiparesis showed differences in positions, with feet together, as most unloaded the weight in the forefoot region, while with feet hip width unloaded in the hindfoot region. As for the assessment by BBS, the functional activities that needed more support obtained lower averages; however, there was no statistically significant correlation with the positions on the board. Conclusion: Health assessment using the low-cost plank can be an important tool to identify changes in balance and carry out appropriate interventions, however it does not correlate with Berg Balance Scale.
